Description

24/7 Crisis Outreach is a non-profit organization which responds to psychiatric crisis in Suffolk County.  We have a hotline available 24/7 as well as a team including a social worker and a nurse practitioner to do home visits in order to conduct interventions in the midst of a crisis.  Our office is located in Port Jefferson, New York though we have numerous people on-call at variable locations throughout Suffolk County in order to increase the community’s accessibility to our services. 

Program

The program provided by 24/7 Crisis Outreach is a combination of a mental health crisis hotline and crisis community outreach.  Both our hotline and community outreach is accessible 24/7. Suffolk County is divided into 3 catchment areas 1. Huntington, Babylon, Smithtown, Islip 2. Brookhaven  3. Riverhead, South Hampton and a team, which includes a social worker and nurse practitioner, is on call in each area and responsible for responding to psychiatric crisis in their respective catchment area.  For example, when a call comes into the hotline a social worker will do a screening.  If a team needs to respond the team in the appropriate catchment area will be paged and then respond to the site of the crisis. If the team feels their safety is in jeopardy they will call 911.  Some optimum outcomes could be an intervention, such as counseling, a referral for the client to follow up with the next business day, visiting a psychiatric emergency room, transported by a friend or family member, by the team collaborates with the hospital staff


Mission

To provide assessment, crisis intervention, and referrals to people in psychiatric crisis within Suffolk county. 

 

To link people in the community with emotional disorders, to appropriate services including hospitalization, out- patient mental health centers, case management, or substance abuse services. 

 

To participate in a collaboration of care for clients served by building relationships with community hospitals, mental health treatment centers, psychiatric emergency rooms, substance abuse centers, and the criminal justice system.
